Smoking Attributable Hospital Treatment in the Czech Republic in 2002 and Estimate of Treatment Costs

OBJECTIVES: To estimate smoking-attributable hospitalizations and to quantify costs of hospital treatment in the Czech Republic in 2002 based on data from the General Health Insurance Company (GHIC).METHOD: For computation of smoking-attributable fractions we used data from the US Cancer Prevention Study II and smoking prevalence from a large national population survey carried out in 2002. Gender and disease specific data on hospitalizations and cost of treatment were provided by the GHIC.RESULTS: In 2002 there were in sum 145,336 smoking attributable hospitalizations. Passive Smoking and Lung Cancer

Passive smoking, or inhalation of tobacco smoke in ambient air, contributes to increased risk of lung cancer in nonsmokers. There is 20 %-30 % higher risk of lung cancer in nonsmokers exposed to passive smoking as compared to those not exposed. Part of IARC multicenter case-control study in countries of central and Eastern Europe has involved determination of exposure to passive smoking by nonsmokers. The odds ratio was calculated by logistic regression adjusted to gender and passive smoking in both the indoor and occupational environments.

Occupational Exposure to Ionizing Radiation and Lung Cancer Risk

The study evaluates the occupational risk of lung cancer from ionizing radiation during work with radioactive substances in subjects undergoing preventive oncological check-ups in district Šumperk. A case-control study of 998 lung cancer cases and 37,772 controls has yielded result of raw OR 6.16 (95% CI 3.85-9.79), which is statistically significant. After adjustment to smoking, OR in smokers was 3.21 (95%CI 1.91-5.32) and 12.33 in non-smokers (95%CI 3.02-41.91). Results demonstrated that risk posed by ionizing radiation was not influenced by smoking.

The relationship between socio-economic status (SES) and health is a frequent subject of epidemiological studies in various populations. Adjustment for SES factors has become a routine component of epidemiological analyses. Health is affected by SES factors both its material and also psycho-social aspects.
